Why are we in South Sudan? In 2026, South Sudan faces a severe humanitarian crisis driven by multiple, overlapping challenges: protracted conflict has displaced millions and disrupted markets, health services, and education.

Humanitarian needs keep increasing as donor funding continues to decline

South Sudan is second‑to‑last in the world in terms of human development as measured by the Human Development Index, ahead of only a couple of other very low‑scoring countries. 

Concern operates in both Northern Bahr eh Gazal State and Unity State as these states face severe food insecurity, widespread displacement, fragile health systems and heightened protection risks compounded by recurring climate and conflict shocks. Our work is critical in providing assistance to the most vulnerable communities.

Acute food insecurity and malnutrition affect over half the population, with millions at crisis or emergency levels; fragile health systems struggle to respond to disease outbreaks such as cholera, measles, and malaria; climate shocks, including floods and drought, destroy homes, crops, and infrastructure; economic collapse and hyperinflation make basic goods unaffordable; and protection risks, especially for women and children, heighten vulnerability to violence, exploitation, and recruitment by armed actors.

In addition, the first-ever presidential elections in December are likely to trigger localised tensions and security challenges, further straining humanitarian access and government capacity to respond to critical needs.

Health & Nutrition

In 2025, Concern expanded health and nutrition services through mobile teams and delivered lifesaving services to conflict and flood affected areas, including communities and refugees from Sudan with no prior access to formal healthcare.
